The Project Request Due Diligence Form is meant to be used to submit a request for a new project or for a modification to an existing application, and also provides the Directors the information needed to make an informed decision about whether to move forward with the request. By completing this document, requestors will be able to establish validity for the request in question, provide details about the resources to be assigned, and share the timeframe within which the request must be completed. 
 
Requests for modifications to existing applications, new reports, or new projects should never be submitted to the Division of Technology & Innovation (DTI) Help Desk nor to the Business Analysts without first receiving executive approval.

You will need to provide a detailed description of the request, including details on why the development is needed, what it will achieve, and how it fits in with current processes.
